Prescott Crash Health Directive saved mom

Lisa called me from YRMC on a Tuesday I’ll never forget.

Her 28-year-old son, Jake, had crashed his Jeep on Highway 89.

He was stable — but unconscious.

His Jeep sat in impound.

State Farm wouldn’t speak to her.

Prescott PD wouldn’t release the report.

Even the hospital couldn’t follow Jake’s wishes.

No one had permission to act.

Jake was 26. He thought health directives were for “grandma.”

But Prescott has deer, ice, and distracted drivers.

One crash — and his world froze.

I drove to the hospital with a blank Arizona Health Directive.

While Jake slept, Lisa and I filled it out.

She named herself as his voice.

She chose his end-of-life wishes.

She added a Power Of Attorney to handle the Jeep, insurance, and bills.

Twenty-four hours later, the Jeep was out.

State Farm took her call.

The police report arrived.

The hospital followed Jake’s plan.

Lisa cried — but this time, from relief.

One free form turned a nightmare into a plan.

Don’t wait for the call.
